How to become an associate member

PSSLAI treats its members not just as customers but also as a family.

Requirements:

1. One (1) valid photo-bearing ID (see list of accepted valid IDs), and submit a clear photocopy of the ID presented with three (3) specimen signatures;
2. One (1) 1×1 ID picture or Webcam Photo;
3. Certificate of Endorsement for PSSLAI Membership (DOWNLOAD HERE)
4. Proof of Relationship to the Regular Member
  • PSA-issued Birth Certificate (original and photocopy)

  • UMID (Unified Multipurpose ID)
  • SSS ID
  • GSIS ID
  • PVC type Philhealth Card
  • PRC ID
  • Passport
  • PhilSys ID or National ID / printed ePhilID
  • OWWA ID
  • Voter’s ID (new type)
  • OFW ID
  • Seaman’s book
  • Company ID
  • Firearm’s license
  • Student ID
  • Dependent’s ID
5. Fees
  • PhP 10  Membership Fee
  • PhP 140  ID & Passbook
  • PhP 1,000  minimum initial deposit for Capital Contribution (Capcon) Account
